Hepatic Encephalopathy

59-year-old Caucasian male with PMH of liver disease, T2DM, Hypotension, and Hypothyroidism brought to the ER by family who state he’s had a change in mood the last day or so and now appears confused. Patients family reports he had just been treated for a UTI 2 weeks ago, but they never went for follow up after completing the antibiotic. Patients family report he has been sleeping a lot and is more lethargic then normal and does not have good intake. Positive Babinski’s sign on assessment. Patient is jaundiced and presents with ascites along with bilateral lower extremity swelling.
