Scenario Description

Seven-year-old Ethan and his friend Jake were playing unsupervised at Jake’s house when they discovered a 9mm handgun in an unlocked nightstand drawer. Curious and unaware of the danger, they handled the firearm, and in a tragic moment, it discharged, striking Ethan in the right temple. The gunshot sent Jake into shock as he screamed for help, bringing his parents rushing into the room to find Ethan unconscious and bleeding heavily. Panicked, they called 911 while attempting to control the bleeding. Ground EMS arrived within minutes, securing the firearm and assessing Ethan, who had shallow breathing, weak pulses, and signs of a traumatic brain injury. Recognizing the severity, EMS activated air medical transport for rapid evacuation to a Level 1 pediatric trauma center.

Learning Objectives

1. Apply crew resource management (CRM) strategies to enhance team communication, role clarity, and decision-making during a high-acuity pediatric trauma scenario.
2. Perform a systematic and prioritized assessment of a pediatric trauma patient using evidence-based trauma evaluation frameworks.
3. Initiate and manage pediatric trauma protocols tailored to the clinical needs of a 7-year-old patient with suspected severe traumatic brain injury.
4. Implement Pediatric Advanced Life Support (PALS) algorithms accurately during the resuscitation of a pediatric patient in traumatic arrest.
5. Select and adjust ventilation techniques, including advanced airway interventions, to optimize oxygenation and prevent secondary brain injury in a child with critical head trauma.
