Scenario Description

While navigating a debris-choked, narrow alleyway during an urban clearance mission, Sgt Jenkins’ squad hears the high-pitched whine of an incoming enemy First-Person View (FPV) kamikaze drone. The drone maneuvers around a collapsed brick building and detonates in mid-air just meters away from him. The blast shatters nearby windows, showers the alley with concrete shrapnel, and knocks Jenkins violently to the pavement. The explosion is immediately followed by enemy sniper and machine-gun fire originating from the upper floors of a structure at the end of the block.

During this simulation, the learner will respond to a high-threat combat scenario requiring immediate tactical and clinical prioritization while under active enemy fire. Operating strictly within the Care Under Fire framework, the learner must first direct the return of fire to achieve fire superiority and rapidly move the casualty behind hard defensive cover. The learner will assess the casualty’s capacity to remain engaged as an active combatant and direct self-aid if appropriate. If life-threatening external hemorrhage is identified, the learner must apply a CoTCCC-recommended limb tourniquet “”high and tight”” directly over the uniform clothing in under 60 seconds to halt arterial bleeding. This phase also requires extracting the casualty from the hostile event and mitigating ongoing threats to prevent additional wounds.

Learning Objectives

1. Identify life-threatening hemorrhage.
2. Apply a CoTCCC-recommended tourniquet “high and tight” over clothing in under 60 seconds.
4. Differentiate between effective and ineffective hemorrhage control by checking for a distal pulse.
4. Extricate the patient to hard cover.
