Atypical Presentation of an Older Adult

Scenario Description

Elderly female in a long term care facility develops acute confusion, diminished appetite, and intermittent complaints of back pain over the past 2 days. The student will have the opportunity to assess the patient, recognize confusion and s/sx of UTI/pyelonephritis, and order point of care testing, including glucose, dipstick U/A, and EKG. The student will then need to present the case in SOAP format, including differentials and a plan, and a present to the collaborating provider. The student will order the antibiotic, labs, and other orders as appropriate.

 

Learning Objectives

Interview patient, long term care facility nursing staff, and/or daughter at bedside

– Perform physical assessment

– Recognize atypical s/sx of UTI

– Order appropriate point of care testing

– Present case with differentials and a plan to collaborating provider

– Order appropriate medication(s) and test after discussion with collaborating provider
