The learner must conduct a head-to-toe assessment, identify environmental safety and fall risks, perform medication reconciliation, and manage a new stage 2 heel pressure injury. The scenario requires navigating complex family dynamics, providing caregiver education on symptom management (shortness of breath/pain), and coordinating care through provider communication.

Learning Objectives

1. Demonstrate a comprehensive hospice home assessment, including a head-to-toe physical exam and environmental safety survey.
2. Identify and mitigate home safety hazards.
3. Execute accurate medication reconciliation and educate the caregiver on the medications for symptom management.
4. Assess and provide initial intervention for a new skin impairment while educating on pressure injury prevention.
5. Demonstrate effective communication with the caregiver and the interprofessional team to coordinate hospice resources.
