Scenario Description

A 78-year-old woman with a history of a stroke is currently admitted with a GI bleed. The learner does a basic nursing assessment including a neurologic exam consistent with previous deficits related to the stroke and leaves the room. While they are absent the patient falls, which requires the learner to evaluate the patient’s mental status and assess any injuries. Using the SBAR format, the learner will communicate all findings to the physician.

– Perform a full assessment after a patient falls
– Communicate findings using the SBAR format
