Scenario Description

A 46-year-old man with a history of prosthetic mitral valve replacement presents to the emergency department with fevers and hypotension. The Learner will identify their diagnosis on rounds and guide the team’s treatment plan as well as communicate that plan with the patient’s family.

Learning Objectives

1. Learner is able to collect information necessary to identify the patient’s medical problems.

2. Learner is able to assess information in collaboration with the interprofessional team to identify and diagnose a patient with infective endocarditis.

3. Learner is able to contribute patient-specific medication-related expertise as part of an interprofessional care team to develop and implement a 4. pharmacotherapy care plan.

4. Learner is able to monitor and evaluate the safety and effectiveness of the care plan.

5. Learner is able to effectively educate the interprofessional care team and the patient as the pharmacotherapy expert.
