TCCC Penetrating Trauma

Case Description

Set in a field environment, participants evaluate a casualty who suffered multiple gunshot wounds. The location is now secure. Participants should perform a MARCH survey, decompress tension pneumothorax, place leg tourniquet, and apply a foley to a junctional axillary wound.

Learning Objectives

1. Provide tactical field care to a casualty at the point of injury.
2. Triage casualties as required.
3. Evaluate for massive hemorrhage and control all sources of bleeding.
4. Perform an initial assessment for hemorrhagic shock including evaluating mental status and palpating radial pulse, and consider immediate initiation of shock resuscitation efforts.
5. Evaluate a casualty’s airway, obtain vital signs including heart rate and pulse oximetry, and perform indicated airway interventions.
6. Evaluate respirations and breathing, and perform interventions as indicated.
7. Evaluate circulation and perform interventions as indicated.
8. Perform hypothermia prevention and place an HPMK.
9. Provide analgesia to the casualty, including PO and parenteral preparations.
10. Provide antibiotics as indicated based on traumatic injuries.
11. Evaluate wounds and place appropriate dressings for wound care.
12. Perform assessment and management despite distractions such as rain or sandstorm, and be able to perform in low light conditions.
