Community Health Worker Program

A Community Health Worker (CHW) or Community Health Coach (CHC) will collaborate with the medical team. Afterwards, the CHW/CHC will call the patient to review the doctor’s care plan, and will then give a presentation and answer questions on basic health maintenance topics – mental health, high blood pressure, and high glucose levels.

Learning Objectives

– Students will demonstrate coordination services for a client with other health providers – communicate with interprofessional team

– Students will demonstrate outreach skills in setting up a presentation

– Student will demonstrate effective communication during presentation

– Student will demonstrate client-centered counseling

– Learner will display motivational interviewing during one-on-one patient interaction

– Student will provide direct services to clients
