Bradley Wright is a 67-year-old African-American male with a history of chronic heart failure. He has had multiple hospital admissions for heart failure. Home healthcare was referred after his last hospitalization for teaching and monitoring of medication and weight. During this scenario, learners must manage the patient’s situation and provide any relevant interventions.
