Scenario Description

Victoria Gonzales is an 8.5-month-old female infant patient discharged to home 4 weeks ago with a tracheostomy tube, g-tube, and an ostomy. Victoria lives in a lower-income 2-bedroom apartment in Phoenix with her parents and 4 siblings (ages 10, 8, 5, and 3) with her mother being the primary caregiver. English and Spanish are spoken in the home. Learners will use appropriate techniques to assess client needs, communicate with caregivers, and deliver client-centered care.

Learning Objectives

1. Assess the Infant’s interprofessional healthcare needs in the home setting, including the family’s physical, emotional, educational, and support systems.
2. Integrate select Interprofessional Education Competencies (IPEC:2023) among the care team to deliver client-centered care.
3. Maintain safe care delivery, including infection control and environmental safety in the home, communicating supportive corrective interventions with caregivers.
4. Plan appropriate interventions with caregivers for home health care and developmental needs and include interprofessional resources.
5. Display professional behaviors and leadership characteristics appropriate for innovative designers, providers, managers, and care coordinators.
6. Support family dynamics that support pediatric patients’ home health care with awareness of cultural and linguistic competence.
7. Implementation of developmentally appropriate communication skills and integration of interprofessional resources.
