Scenario Description

A patient, recently diagnosed with heart failure during an inpatient stay for respiratory distress and pneumonia, presents to the outpatient clinic. He was discharged two weeks ago with new medications that he has not yet filled or taken. The patient does not understand the necessity of the medication and demonstrates a significant educational need regarding his diagnosis and the ongoing care required to maintain optimal health. During this visit, the learner will assess and educate the patient on his diagnosis, medications, lifestyle modifications, and dietary recommendations, followed by an evaluation of the educational effectiveness.

This scenario has been designed for a moderatorless experience.

Learning Objectives

1. Analyze the patient’s physical status, motivational readiness, and specific barriers to compliance during the initial outpatient assessment.
2. Apply evidence-based communication strategies to educate the patient on heart failure pathophysiology, essential medications, and prescribed lifestyle modifications.
3. Evaluate the patient’s understanding of their chronic care plan by executing a structured teach-back verification and an objective knowledge assessment.
