Case Description

In this scenario, learners will explore an interprofessional healthcare team tasked with developing a holistic care plan for Olivia Buffington, a 55-year-old woman referred to the Wellness Clinic for healthy lifestyle modifications following recent lab work. Learners review patient documentation and perform assessments, mirroring real-world practice. Each learner, representing their unique profession, will individually enter Olivia’s exam room to conduct assessments and engage with her. This experience will challenge learners to analyze their own professional role and the roles of others while communicating effectively with both Olivia and the interdisciplinary team. The scenario culminates in a crucial interdisciplinary team meeting, where learners will collaborate to synthesize their findings, evaluate potential interventions, and establish a shared plan of care, ensuring they foster a climate of mutual respect and articulate their personal values and ethics. The team will collectively return to Olivia’s room to present their collaborative plan, demonstrating professional behaviors and leadership characteristics to provide patient-centered care.
