Scenario Description

An 83-year-old female was admitted to a high-acuity Skilled Nursing Facility (SNF) 25 days ago after experiencing a significant left-sided ischemic stroke. She has significant right-sided deficits and requires assistance completing Activities of Daily Living (ADLs). In this scenario, learners must assist a dependent patient in completing daily cares, including nutrition assisting, oral care, bed bathing and perineal care, administering medications, and communicating with the interprofessional team.

Learning Objectives

1. Learners will demonstrate daily care activities, including nutrition assistance, oral care, bed bathing, and perineal care for a dependent patient.
2. Learners will prioritize patient care activities in the context of the patient’s plan of care.
3. Learners will apply evidence-based principles for safe medication administration.
4. Learners will communicate with the patient and interprofessional team to promote self-care and coordinate healthcare delivery.
