Scenario Description

A 56-year-old female was admitted to the oncology unit 8 days ago for chemotherapy management and supportive care due to treatment complications following a recurrence of breast cancer. She has a past history of a right-sided mastectomy with axillary lymph node removal. She has a left-sided tunneled central venous catheter for chemotherapy and a left-hand peripheral IV for supportive therapy. After completing her most recent chemotherapy dose, she is being treated for hypokalemia and requires close follow-up monitoring. Because of the high risk of neutropenia with her current chemotherapy regimen, she is maintained on neutropenic precautions throughout her hospitalization. In this scenario, learners must provide safe care for a patient on an oncology floor, follow appropriate precautions, administer treatments, and collaborate with the interprofessional team.

Learning Objectives

1. Integrate patient preferences and non-pharmacologic interventions to promote comfort and individualized care in a chemotherapy patient.
2. Identify and respond to clinical findings consistent with fluid and electrolyte imbalance, including signs and symptoms of hypokalemia.
3. Demonstrate principles of central venous catheter (CVC) safety, including aseptic technique, line access precautions, and dressing change protocols.
4. Apply effective interprofessional communication techniques to advocate for patient needs related to sleep, pain management, and electrolyte imbalance.
